Describes the Adherence Badge and Trend Graph features of the Patient Profile.
For Premium and RCS customers, using the nightly data from your PMS, the Outcomes patient profile can determine if a patient qualifies for 1 of the CMS Adherence measures (Diabetes, Hypertension, Cholesterol). For patients that qualify, you will see Adherence badge(s) on the top right of the patient profile.
- DM = Diabetes
- HTN = Hypertension
- CHOL = Cholesterol
How does it work?
- When a patient has at least 2 fills of a medication class for a respective measure(s), the Adherence Badge(s) will appear in the top right navigation bar of the patient profile.
- If the patient has enough fills (2) to calculate a PDC for the Current Year, the patient's current year PDC value will be displayed on the Adherence Badge(s).
- If the patient does not yet have enough fills to calculate a PDC for the Current Year but had enough fills in the Previous Year, the Adherence Badges will display DM/HTN/CHOL but with a dash (-) to indicate that they do not yet have a value for the current year but did qualify in the previous year
- You may then click on the badge(s) to view the Adherence Trend Graph for that measure which will show more data regarding medications, dates, percentages, etc.
What is an Adherence Trend Graph?

- Within the Adherence Trend Graph, the user will see more information on the Current Year and Previous Year tabs such as the medications that contributed to the measure along with data such as Rx #, Date Filled, NDC, Refills, Fill Number, Quantity, Days Supply
- When the patient has a current year and previous year PDC, when clicking on the adherence badge, the user will be brought to the Current Year tab of the Adherence Trend Graph by default.
- When the patient does not yet have enough fills in the current year to calculate a PDC (2 fills) but did have a PDC for the previous year, when clicking on the adherence badge, the user will be brought to the Previous Year tab of the Adherence Trend Graph by default.
- The user will also see a trend graph to show how the patient is trending towards an adherence goal of 80% with a Last Possible Fill date that the patient can work towards to achieve that 80% goal.
- The user can toggle between Current Year and Previous year data.
- Note: When there are not enough fills to qualify for a measure, a message will display to explain "At least two medication fills are needed to calculate a PDC score."
