Frequently Asked Questions about Outcomes eCare Plans
What is an eCare Plan?
An eCare Plan is a structured, electronic documentation standard that allows pharmacists and other healthcare providers to record, share, and coordinate patient care activities.
Within Outcomes, the eCare Plan tool enables pharmacies to document patient assessments, clinical interventions, care goals, medication-related recommendations, and follow-up activities in a standardized format. eCare Plans help support care coordination, quality initiatives, and value-based care programs by capturing clinical services in a consistent, interoperable manner.
What information is included in an eCare Plan?
An eCare Plan may include information such as:
- Patient demographics
- Medication-related concerns
- Clinical assessments
- Goals of care
- Care interventions
- Follow-up activities
- Health observations and outcomes
The information captured will vary based on the clinical service being documented.
How much does the Outcomes eCare Plan tool cost?
The Outcomes eCare Plan tool is included at no additional cost for pharmacies using the following Outcomes pharmacy management systems (PMS):
- Rx30
- Computer-Rx
If your pharmacy uses Rx30 or Computer-Rx and does not currently have access to the eCare Plan tool, please contact Outcomes Support for assistance enabling the feature.
Can I use the Outcomes eCare Plan tool if I do not use Rx30 or Computer-Rx?
Yes. The Outcomes eCare Plan tool can be used with other pharmacy management systems in addition to Rx30 and Computer-Rx. For pharmacies using a different PMS, the eCare Plan tool is available through the Outcomes Premium Clinical Module.

How does Outcomes receive patient information for eCare Plans?
Outcomes receives patient data through automated nightly data feeds from participating Pharmacy Management Systems. These data feeds help keep patient information current and ensure eligible patients can be located when documenting an eCare Plan.
What patients can I create an eCare Plan for?
Outcomes receives patient data through nightly data feeds from your pharmacy management system (PMS). In general, any patient who has had a prescription dispensed by your pharmacy within the past 12 months should be available and searchable within the Outcomes eCare Plan tool.
If you are unable to locate a patient you expect to see, verify that:
- The patient has received a fill or dispense from your pharmacy within the past 12 months.
- Your pharmacy's PMS data feed is active and transmitting successfully.
- The patient record has had sufficient time to be included in the nightly data update.

Does the Outcomes eCare Plan support interoperability?
Yes. The Outcomes eCare Plan is designed to support healthcare interoperability through the use of industry-standard formats and terminology. Today, Outcomes supports the exchange of eCare Plan information using:
- FHIR-based JSON
- CCD (Continuity of Care Document)
The Outcomes eCare Plan also leverages industry-standard clinical terminology, including:
- SNOMED CT for clinical concepts and conditions
- LOINC for laboratory and health-related observations
Does Outcomes automatically send eCare Plan data to other healthcare systems or organizations?
Currently, Outcomes automatically transmits eCare Plan data only to CPESN (Community Pharmacy Enhanced Services Network) for pharmacies participating in eligible CPESN programs.
At this time, Outcomes has not enabled automated eCare Plan transmission to other external organizations or healthcare systems. Pharmacies may still access and utilize eCare Plan documentation within Outcomes, including available export formats, but automated transmission is currently limited to CPESN-supported workflows.
How often is eCare Plan data sent to CPESN?
Outcomes transmits eCare Plan data to CPESN on a daily basis for eligible participating pharmacies.
How does eCare Plan transmission to CPESN work?
For pharmacies participating in CPESN programs:
- CPESN provides Outcomes with a monthly file of participating pharmacy NPI numbers.
- Outcomes uses those NPI numbers to identify eligible pharmacies.
- eCare Plan data is transmitted to CPESN daily.
- Only pharmacies identified through the CPESN participation file are included in automated transmissions.
How do I enroll in CPESN eCare Plan transmission?
To begin sending eCare Plan data from Outcomes to CPESN, the following steps must be completed:

- Contract with CPESN - The pharmacy must first enroll and contract directly with CPESN.
- Complete the CPESN Data Attestation Form - This form authorizes Outcomes to transmit eCare Plan data documented in the Outcomes eCare Plan tool to CPESN on the pharmacy's behalf.
- CPESN Adds the Pharmacy to Their Roster - CPESN provides Outcomes with an updated roster of participating pharmacies. The roster includes participating pharmacy NPIs and is used to identify pharmacies eligible for data transmission.
- eCare Plan Transmission Begins - Once the pharmacy appears on the CPESN roster, Outcomes will begin transmitting eligible eCare Plan data to CPESN as part of the daily transmission process.
Does documenting an eCare Plan automatically qualify my pharmacy for reimbursement or participation in a payer program?
No. Creating or transmitting an eCare Plan does not guarantee reimbursement or participation in a payer program. Program requirements are determined by the payer, network, or program sponsor.

Is participation in CPESN required to use the Outcomes eCare Plan?
No. Pharmacies can use the Outcomes eCare Plan tool regardless of CPESN participation. However, automated eCare Plan transmission is currently limited to pharmacies participating in eligible CPESN programs.
