Getting Started with MTM Services: A Guide for Pharmacies Exploring MTM Opportunities

Whether you're exploring MTM for the first time or looking to build MTM momentum, this actionable to-do guide will walk you through how to get started, build confidence, and set your team up for success.  
 
To patients, MTM services provide meaningful benefits by helping catch or prevent potential medication issues, improving their understanding of how and why they take their medications, and reinforcing that someone is genuinely invested in their health and well-being. 

To the pharmacy, MTM services offer valuable advantages by fostering stronger relationships with patients, generating revenue through clinical services, and enhancing overall job satisfaction for staff by allowing them to practice at the top of their license and make a greater impact on patient care.

Prepare Your Team for MTM 

Ensure everyone supporting MTM: 

  • Has their own login credentials (Pharmacists, Techs, Interns) and pass the required online training
    • Create an account at secure.outcomesmtm.com 
  • Knows MTM service types 
    • CMR – Comprehensive Medication Review 
    • TIPs – Targeted Intervention Program for a specific medication or service 
  • Agrees to comply with the current Outcomes Policy and Procedure guide that outlines requirements for providing and documenting MTM services in Outcomes programs 
  • Understands services must include direct communication with eligible patient and/or prescriber as applicable 
  • Documents service outcomes reflective of actual interventions 
  • Is familiar with any MTM specific processes or protocols at your MTM Center 

Share Best Practices: 

  • Start at the MTM Opportunities queue on the Dashboard, working patients top to bottom 
  • Most services can be completed in person or by phone 
  • Leave work in progress in pending status and schedule planned CMRs 
  • Involve your entire team and assign roles based on strengths  
  • Leverage MTM integration with your dispensing system when available  
  • Focus on completing the essential requirements of each service efficiently while still delivering quality care 
  • Ensure one team member is set up with Pharmacy Administrator access to manage users and view MTM reporting  

Define Roles and Responsibilities

Pharmacists Technicians Students/Interns
Facilitate and complete CMRs Schedule CMRs Shadow the pharmacist

Focus on TIPs that can only be done by pharmacists

Complete CMR pre-work as needed Support like a technician
Provide counseling to patients Notify team members of incentives/bonuses Provide services under pharmacist supervision aligned to their level of training
Make recommendations to prescribers Complete technician-eligible TIPS  

Track Progress

  • Assign individual goals  
    • Ensure at least one team member logs in daily to monitor eligible patients and act on opportunities. More team members may need to be involved daily depending on the number of opportunities available.  
    • Example goals (align to your pharmacy’s volume): 
      • Techs: 2–3 TIPs/day 
      • Pharmacists: 5 CMRs/week + 10 TIPs/week 
  • Track goals, milestones and MVPs 
  • Example pharmacy goals:  
    • Achieve performance score of 3 or above 
  • Give regular updates and celebrate progress 
  • Pharmacy Administrators can leverage reporting to monitor goals
