Outcomes Adherence Monitoring Program (AMP)

Adherence Monitoring Program (AMP) is a longitudinal service designed to support patients in maintaining or achieving a target medication adherence rate by the end of the calendar year. Patients enrolled in this service are typically non-adherent or identified as at risk of becoming non-adherent. If the pharmacy completes the initial enrollment TIP, all required quarterly checkpoint TIPs, and the patient meets the targeted adherence rate (80%+), the pharmacy will qualify for a bonus.

Two Key Steps to Complete the Service

Step 1: Adherence Monitoring Enrollment TIP  

This is the initial discussion with the patient where you’ll verify that the patient is still taking the targeted medication or another medication in the same drug class and discuss the proper usage and importance of taking the medication as prescribed. During the discussion, identify any current or potential barriers to adherence and offer appropriate solutions before submitting the TIP through the Outcomes platform. You may also want to let the patient know that you’ll be checking in again later in the year to ensure the medication is still working well for them. 

Step 2: Adherence Monitoring Checkpoint TIPs  

Quarterly checkpoint TIPs populate following successful completion of an enrollment TIP. Contact the patient to reinforce the purpose of this medication and discuss the importance of adherence. Use the DRAW tool to assess if the patient is still experiencing adherence barriers and offer solutions to overcome them if identified. Complete these TIPs and monitor PDC rates throughout the year for your pharmacy to be eligible for a year-end bonus. Depending on the patient’s progress, more frequent monitoring may be necessary. 

Patients may become disenrolled for AMP if: 

  • An Adherence Monitoring Checkpoint TIP is not successfully completed. 
  • The target adherence rate, typically 80% PDC, is no longer achievable within the calendar year.

FAQ: 

Q: Do I have to speak with the patient to perform an Adherence Monitoring Checkpoint TIP? 
A: Yes. To complete a checkpoint TIP, you must speak with the patient. A chart review on its own does not meet the requirements. 

Q: Is a refill required as part of this TIP? 
A: No. A refill isn’t required for this TIP, though you may complete one if it supports the patient’s needs. 

Q: How do I know if the patient is still able to achieve the targeted PDC?  
A: As long as the patient is still targeted for the checkpoint TIP, they are still able to achieve the targeted PDC. PDC rates are also available under the patient's adherence status within the TIP. 

Q: How often are the metrics including # fill days missed & % adherence found in the “Adherence status” within the TIP getting updated?  
A: As we receive data from our health plans, this information is updated. The frequency varies by health plan but can range from daily to once a month. 

Q: Can technicians complete these TIPs? 
A: No. A pharmacist is required to complete these TIPs because the adherence conversation involves applying clinical judgment and educating the patient on relevant clinical information. 

Q: If my pharmacy completed the initial Adherence Monitoring TIP, will the quarterly Adherence Monitoring Checkpoint TIPs still appear at other pharmacies?  
A: No, checkpoint TIPs will only appear at the MTM Center that completed the initial Adherence Monitoring enrollment TIP. 

Q: How long is an Adherence Monitoring Checkpoint TIP available? 
A: Adherence Monitoring Checkpoint TIPs start to generate the quarter after the initial Adherence Monitoring enrollment TIP is completed. Checkpoint TIPs will not appear until 30 days have passed from Adherence Monitoring enrollment TIP completion date. So in most cases, pharmacists have 60–90 days to complete a checkpoint TIP.  

Q: What if the patient’s medication dose has changed?  
A: If the prescriber changes the dose of the medication included on an Adherence Monitoring Checkpoint, the patient is still eligible for the checkpoint TIPs and bonus. We will update the medication once we see it in prescription claims.
