How to register your pharmacy with the Alaska (AK-VacTrAK) Registry
Step 1: Complete Enrollment Forms
Complete the following enrollment forms for AK-VacTrAK:
- VacTrAK New Provider application
- HL7 Data Exchange
- VacTrAK Healthcare provider contract
- VacTrAK Authorization to Add / Modify Users
- VacTrAK Interface Project Profile Form
Step 2: Send Confirmation Email to Outcomes
Please send the completed forms directly to the IIS team at pwiisforms@outcomes.com:
- Subject line: <Pharmacy Name> /Vaccinations / AK Registry Enrollment Forms
- Body of email: Please include the below
- Attached forms
- Pharmacy name, NPI/NCPDP, and address
- Primary contact information
Step 3: Document Vaccines in AK-VacTrAK and Outcomes
Please continue to report immunizations to AK-VacTrAK and update any tasks in Outcomes while we work on the connection to the state registry.
Step 4: AK-VacTrAK Requires a Testing Phase
The IIS team at Outcomes will assist in the Live Testing based on your documented, administered vaccines. Please document all administered immunizations and update any tasks in your Outcomes Portal. Testing requires 100% error-free messages.
Step 5: Final Steps
Once your pharmacy is in production, the IIS team will send you a confirmation email with your go-live date.
