1. In order to move forward with enrollment please complete the form by visiting the following link: Colorado Public Health Reporting (CoPHR) Portal

• Please select Pharmacy & fill out the form provided.

2. Please send an email to iisforms@prescribewellness.com with the subject line: <Account Name> / Vaccination Tool / CO Public Health Reporting Complete. A Pharmacy Integration Manager will reach out to the registry thereafter.

Step 2: Documenting Immunizations manually

In CO-CIIS and in the Outcomes Vaccinations Tool:

• Please continue to report immunizations to CO-CIIS while Outcomes is working on the pharmacy’s integration USIIS User Confidentiality and Security Agreement

Step 3: Data Testing

CO-CIIS requires data testing and will schedule a kickoff call to begin the integration.

Step 4: Final registration steps

CO-CIIS Status Check:

• CO-CIIS will provide testing credentials to move your pharmacy into active testing
• CO-CIIS will review your most recent vaccines and check to see that all required information is inputted and submitted properly
• Once CO-CIIS approves, they will schedule a closing call to finalize the integration and confirm your pharmacy's Production status
