How to register your pharmacy with the Connecticut (CT-WiZ) Registry
Step 1: Enrollment
Go to the CT-WiZ portal and complete the HL7 Application:
- Complete the CT WiZ HL7 Application Form HERE
- Review the first tab in the Application Form and complete the other 4 tabs in the spreadsheet.

Once complete, please save and send a copy of the form to iisforms@prescribewellness.com.
If your pharmacy already has an account with CT-WiZ, please send your Organization Code, Pharmacy Name, Pharmacy System, State (CT), and Contact Information to iisforms@prescribewellness.com.
Step 2: Document Immunizations
- Please continue to document all administered immunizations in your Patient Engagement Center.
Step 3: Final Steps
Once your pharmacy is in production, Outcomes will send you a confirmation email with your go-live date to continue reporting immunizations in your Outcomes Vaccinations tool.
