How to register your pharmacy with the Connecticut (CT-WiZ) Registry
Step 1: Register with CT-WiZ
Go to the CT-WiZ portal and complete the HL7 Application HERE
- Use the following information to review the first tab in the Application Form and complete the other 4 tabs in the spreadsheet:
- Is clinic attesting to Meaningful Use: If pharmacy is or will be accepting publicly funded vaccines, please answer with YES. If all vaccines are privately funded, please select No.
- Does the clinic have a technical lead? No
- Vendor/HL7 and Software Information
- Vendor Name: SMP / PrescribeWellness
- Software Version: 2.0
- Is your EMR/EHR equipped to use HL7 version 2.5.1? Yes
- Clinic's software uses SOAP Web Services: Yes
- Type of interface: Bi-Directional
- Will the interface be reporting directly to CT-WiZ or through a centralized reporting Hub? Reporting Hub-EHR Hub

Once complete, please save and send a copy of the form to iisforms@outcomes.com.
If your pharmacy already has an account with CT-WiZ, please send your Organization Code, Pharmacy Name, Pharmacy System, State (CT), and Contact Information to iisforms@outcomes.com
Step 2: Document Vaccines in CT-WiZ and Outcomes
Please continue to report immunizations to CT-WiZ and update any tasks in the Outcomes platform while our team works on the connection to the state registry.
Step 3: CT-WiZ Requires a Testing Phase
The IIS team at Outcomes will assist in the Live Testing based on your documented, administered vaccines. CT-WiZ registry requires a minimum of 2 weeks of testing. Please document all administered immunizations and update any tasks in your Outcomes portal. Testing requires 100% error-free messages.
Step 4: Final Steps
Once your pharmacy is in production, the IIS team will send you a confirmation email with your go-live date.
