How to enroll your pharmacy with the Florida (FL-SHOTS) Registry
Step 1: Registration
- If you have a FL-SHOTS account, please send your ORG ID, Pharmacy Name, Pharmacy System, and Contact Information to iisforms@prescribewellness.com
- If you do not have a FL-SHOTS account, please complete the FL-SHOTS User Agreement. Please save it and send a hard copy to FL-SHOTS and email a copy to iisforms@prescribewellness.com.
- Optional: If you would like to attest to Meaningful Use, which includes participating in the FL-SHOTS COVID program, please complete the online Interest of Intent.
Step 2: Document Immunizations
- Please continue to document immunizations in your registry portal and the Vaccinations tool while Outcomes works on your pharmacy’s integration.
Note: FL-SHOTS will call the POC to confirm registration and review the application directly. This will be a phone call or an email. If this is not completed by the provider, the integration process will be delayed. This is a State regulatory step.
Step 3: Final Integration Steps
When your pharmacy is approved to Go Live with the state registry for automated reporting, the Outcomes team will send you a confirmation email with your start date. Once your pharmacy is in production, you will no longer need to manually report vaccines on your state registry website.
