Step 3: Documenting Immunizations manually to HI-HIR and in the PrescribeWellness Vaccinations Tool:
- Please document all administered immunizations in the Outcomes Vaccinations Tool as well as your state registry portal.
Step 4: Final registration steps
1. Complete and submit the following two forms via email to iisforms@prescribewellness.com and cc registryhelp@doh.hawaii.gov:
- Annual Facility Enrollment Application - This form must be reviewed and completed by the Physician or the Organization/Site Administrator. One form per facility location is required.
- Confidentiality and Security Statement - This form must be reviewed and completed by each user who has an HIR account to continue accessing HIR. Each user must also initial the top right section of the page after viewing the training video(s).
2. HI-HIIR will provide Outcomes with testing credentials.
3. Once credentials are received, your pharmacy will go into active testing.
4. HI-HIIR will review your most recent vaccines and check to see that all required information is inputted and submitted properly.
5. Once HI-HIIR approves, they will confirm the Production status of your pharmacy.
