Identifying good candidates for MedSync is the first step in establishing a successful program.
Here are two ways to get started:
1. Med Sync Patient List
- Click on Patients within the left-hand navigation menu
- Select Med Sync Eligible from the submenu
A table of patients eligible for synchronization populates.
When starting a Med Sync program, we recommend you narrow down the list of eligible patients to a manageable list. From the Not Enrolled tab, you can filter the list of patients by clicking on the Filters button. You can select multiple filter options. Once you have selected your desired filters, click Apply.
2. Active Patients
To find candidates form the Active Patients list:
- Navigate to Patients from the left-hand navigation bar
- Navigate to Active Patients from the Patients Sub Menu
Here, you will see the performance Quality Metrics. Each disease state is displayed with a percentage of adherence. A patient is considered adherent if their adherence measure is at least 80%.
- Click on the dark portion of a ring to display a list of the patients that are 70-80% adherent.
