Indiana (IN-CHIRP) Registry Enrollment / HL7 Integration
Step 1: Registration
Please complete the following required forms to register with IN-CHIRP:
Please email both forms to PrescribeWellness at iisforms@prescribewellness.com with the following subject line: Pharmacy Name/ Vaccinations/ Attached IN-CHIRP Forms
Step 2: Documenting Immunizations
- Please document all administered immunizations in the Outcomes Vaccinations Tool as well as in your state registry portal
- To request state registry training, please complete this form
Step 3: Final registration steps
- Once credentials are received, your pharmacy will move into Production status
- Once approved, IN-CHIRP will send a confirmation email to your pharmacy
