      How to register your pharmacy with the Kentucky (KY-KHIE) Registry

      Step 1: Complete the enrollment forms

      Please fill out the KY-KHIE Intake Form.

        Step 2: Confirmation email

        1. Find your KY-KHIE Outreach Coordinator based on your county.
          1. IF your county lists Kelly as the Outreach Coordinator, please refer to Ann O’Hara instead at Ann.OHara@ky.gov while KY is undergoing some changes.
        2. Send an email to your Outreach Coordinator and cc iisforms@prescribewellness.com with the below information:
          1. Pharmacy Name
          2. County
          3. EHR: SMP PrescribeWellness
          4. Primary Contact
          5. Phone Number
          6. Pharmacy Address
        3. Your Outreach Coordinator will then contact you to begin the IIS integration process.

        Step 3: Document Immunizations

        • Please continue to document immunizations in the Vaccinations tool while Outcomes works on your pharmacy’s integration.
          • You can also find pre-recorded webinars HERE.

        Step 4: Final Integration Steps

        The Outcomes team will send you an email confirmation when your pharmacy location has been approved to go live to report to KHIE with your Vaccinations tool. Once your pharmacy is in production, you will no longer need to manually report vaccines on your state registry website.
