How to register your pharmacy with the Massachusetts (MA-MIIS) Registry
Step 1: Create an account with your state registry
Complete this step if you do not currently have an MA-MIIS account

Step 2: Complete the Onboard Request for MA-MIIS
After completing registration, you must submit an onboarding request to initiate the process of connecting and submitting data.

Questions for the onboarding request form:
Question 2 – Indicate Onboarding Type - Select the type on Onboarding you are interested in:
Answer: Send Immunization Data and establish Bi-Directional Data Exchange (VXU & QBP/RSP)
Question 6 – Identify Technical Contact:
Answer: Amy Milversted
Answer: Email Address - amilversted@trhc.com
Question 7 – Select your Electronic Health Record:
Answer: SMP / Outcomes
Step 3: Send Onboarding Request Confirmation
Please send a confirmation of your onboarding request to IIS@prescribewellness.com with the following subject line: Pharmacy Name / Pharmacy Management System / Onboarding Request Confirmation
Step 4: Interested in Applying to Administer the COVID-19 Vaccine

Step 6: Manually Report to MA-MIIS while the interface is being worked on
- Please continue to report immunizations to MA-MIIS and in your Vaccinations tool while Outcomes is working on the pharmacy’s integration.
Step 7: Final Integration Step
Outcomes will send an email confirmation when your pharmacy location has been approved to go live to report with the Vaccinations tool to MA-MIIS.
