      How to enroll your pharmacy with the Michigan (MI-MCIR) Registry

      Step 1: Registration

      • If your pharmacy does not have an MCIR account, please register HERE.
      • Next, please email iisforms@prescirbewellness.com to request the Provider Site Usage Agreement with the following subject line:
        • Subject line when requesting for the provider site usage agreement: Account Name | Vaccination Tool | Requesting for Provider Site Usage Agreement
      • Once received, complete the MCIR Provider Site Usage Agreement and return it directly to your MCIR Region (see below) and cc iisforms@prescribewellness.com on the email:

      MCIR ID

      • If you have an MCIR Account, please forward your MCIR ID to iisforms@prescribewellness.com with the following subject line: Account Name | Vaccination Tool | MCIR SITE ID ATTACHED.

      How to Obtain MCIR ID:

      • Log into your MCIR Account
      • Click My Site within the top bar
      • Next, click Edit My Site
      • Your MCIR ID will be in the right-hand corner – MCIR Site ID 1XXXXXXXXXXX

        Step 2: Document Immunizations

        • Please continue to report immunizations to MI-MCIR and in Outcomes.

        Step 3: Final Integration Steps

          Outcomes will send a confirmation email when MI-MCIR has confirmed that your pharmacy is live in production and can document vaccinations in the Vaccinations tool.
