How to enroll your pharmacy with the Montana (MT-imMTrax) Registry
Step 1: Complete forms
1. Please fill out the two required forms HERE and email the completed documents to iisforms@prescribewellness.com.
2. Be sure to review your consent forms and attach them to your intake forms before administering immunizations.
Step 2: Document Immunizations
- Please continue to report immunizations to MT-imMTrax and in the Vaccinations tool while Outcomes works on your pharmacy’s integration.
Step 3: Final Integration Steps
Montana Status Check
- The Montana Registry will provide Outcomes with testing credentials
- Once credentials are received, your pharmacy will go into Active testing
- MT-imMTrax will review the most recent vaccines and check to see that all required information has been submitted properly
- After approval, MT-imMTrax will send you an email to confirm that your pharmacy is ready for Production.
