      How to enroll your pharmacy with the New Mexico (NM-NMIIS) Registry

      Step 1: Registration

      Send an email to the New Mexico Registry requesting to initiate data exchange include Marlene Pena- marlene.pena@doh.nm.gov and cc Katheryn Hernandez- khernandez@outcomes.com

        Step 2: Complete NMSIIS Provider and EHR form

        Complete the provider and EHR form sent from Marlene.

        A.For section EHR Contact Information please add:
        EHR Vendor: Outcomes SMP Version 2.0
        i. First Name: Katheryn
        ii. Last name: Hernandez
        iii. Title: Integration Consultant
        iv. Email address: khernandez@outcomes.com
        v. Phone number: 949-398-4621

        Step 3: Document Immunizations

        • Please continue to report immunizations to NM-NMIIS and in the Vaccinations tool while Outcomes works on your pharmacy’s integration.

        Step 4: Final Integration Steps

        1. NM-NMIIS will reach out to the provider and Outcomes when they are ready to start testing.
        2. Outcomes will send an email confirmation when your pharmacy location has been approved to go live to report with NM-NMIIS
