      How to enroll your pharmacy with the New Mexico (NM-NMIIS) Registry

      Step 1: Registration

      If your pharmacy does not have an account with the New Mexico Registry, please register HERE.

        Step 2: Submit forms

        Please fill out the below agreements and email a copy of your completed forms to iisforms@prescribewellness.com with the following subject line: Pharmacy Name / Vaccination Tool / NM-MIIS Survey Completed.

          Step 3: Document Immunizations

          • Please continue to report immunizations to NM-NMIIS and in the Vaccinations tool while Outcomes works on your pharmacy’s integration.

          Step 4: Final Integration Steps

          1. NM-NMIIS will reach out to the provider and Outcomes when they are ready to start testing.
          2. Outcomes will send an email confirmation when your pharmacy location has been approved to go live to report with NM-NMIIS
