How to enroll your pharmacy with the New York State (NYSIIS) Registry
Step 1: Registration
Please complete the below forms to enroll:
- NYSIIS Instructions
- NYSIIS User Agreement
- Be sure to fill out all information and include your pharmacy's main point of contact
Step 2: Confirmation Email
Once complete, please email your completed forms to iisforms@prescribewellness.com with the following subject line: Pharmacy Name/SMP/NYSIIS Enrollment Completed. Please include your pharmacy details and confirmation that you have completed the NYSIIS enrollment steps above in the body of the email.
Step 3: Document Immunizations
- Please continue to document immunizations in the Vaccinations tool. At this time, our team will work with NYSIIS to set up your pharmacy’s credentials and connection feed.
Step 4: Final Integration Steps
Outcomes will notify your pharmacy with a confirmation email once your pharmacy has been approved for production and is live to report to NYSIIS with the Vaccinations tool.
