How to register your pharmacy with the North Carolina (NCIR) Registry
Step 1: Complete NC-NCIR Forms
Complete the following 3 forms:
- North Carolina Immunization Registry Pharmacy Agreement
- NCIR User Confidentiality Agreement
- NC HIEA Full Participation Agreement for Pharmacies
Note: If your pharmacy does not have a physician, the pharmacist-in-charge may sign the forms instead.
Step 2: Send the Completed Forms to NCIR
Send a copy of the completed forms to NCIR using one of the below methods:

Step 3: Send the Completed Forms to Outcomes
Send a copy of the 3 completed forms via email to pwiisforms@outcomes.com
Step 4: Document Vaccines in NCIR and Outcomes
Please continue to report immunizations to NCIR and update any tasks in the Outcomes platform while our team works on the connection to the state registry.
Step 5: Production
When your pharmacy is approved to Go Live with the state registry for automated reporting, the IIS team will send you a confirmation email with your go-live date. Once your pharmacy is in production, you will no longer need to manually report vaccines on your state registry website
