How to register your pharmacy with the North Carolina (NCIR) Registry
Step 1: Complete the enrollment forms
Note: If your pharmacy does not have a physician, the pharmacist-in-charge may sign the forms instead.
Step 2: Send the Completed Forms to NCIR
Send a copy of the completed forms to NCIR using one of the below methods:

Step 3: Send the Completed Forms to PrescribeWellness
Send a copy of the 3 completed forms via email to iisforms@prescribewellness.com.
Step 4: Switch from CVMS to NCIR to manage inventory
- Visit https://covid-enroll.ncdhhs.go... and click “Register”
- Complete the registration form and click "Sign Up"
- In Section A of the Provider Enrollment process, the Organization Administrator will enter:
- Organization details
- Organization administrator contact details
- Locations administering vaccines
- Executive Officer (CEO and CMO) information: It is helpful for Organization Administrators to collect all of this information before continuing on to complete Section A
- After entering organization and location details, the Organization Administrator will select NCIR as the preferred system for managing COVID-19 vaccines.
- If NCIR is selected, the Organization Administrator will be prompted to complete the NCIR agreement. From the drop-down, select “NCIR.” Then agree to the terms of the NCIR agreement and click “Next.”
- To complete Section A, the Organization Administrator will enter in the Responsible Officer (CEO and CMO) information and request their signatures.
Step 5: Document Immunizations
- Please continue to report immunizations to NCIR and in the Vaccinations tool while Outcomes is working on connection to the state registry
Disclaimer: The NCIR registry is ready to connect with Outcomes to electronically report all vaccines EXCEPT COVID-19 vaccines. COVID-19 specific vaccines must be reported via the CVMS portal manually. In North Carolina, COVID-19 vaccines are currently not available for electronic reporting via vendors.
Step 6: Production
When your pharmacy is approved to Go Live with the state registry for automated reporting, the Outcomes team will send you a confirmation email with your go-live date. Once your pharmacy is in production, you will no longer need to manually report vaccines on your state registry website.
