      How to register your pharmacy with the Ohio OH (ImmpactSiiS) Registry

      Step 1: Registration

      1. Register for an IMPACTSIIS account

      • If you already have an ImpactSIIS account, please skip step 1 and go to step 2
      • If the pharmacy is not registered with ImpactSIIS, please register HERE

      2. Sign and complete the OH-ImpactSIIS Security Agreement attached to this email. Then follow the steps below:

      • Link to OH-ImpactSIIS Security Agreement
      • Complete the attached form and send it back to iisforms@prescribewellness.com. Please make sure you include your Pharmacy Name, VaccineComplete Integration, State, and Contact Information in the email.
      • Physically mail the original form with your signature to the Ohio Department of Health Immunization Program, ImpactSIIS to the below address:

        Step 2: Document Vaccines in Your Patient Engagement Center

        Please document all administered immunizations in Outcomes.

        Step 3: Production

        After the state registry approves your location to move forward into Production, the Outcomes team will send you a confirmation email with your go-live date. Once your pharmacy is in production, you will no longer need to report vaccines on your state registry website manually.
