How to enroll your pharmacy with the Oklahoma (OK-OSIIS) Registry
Step 1: Registration
Please complete the below enrollment forms:
- OSIIS Authorized Site Agreement (ASA)
- COVID Pandemic Provider Enrollment
- Click Providers and select Enrollment, then Enroll Now
Once complete, please send an email to iisforms@prescribewellness.com to confirm that you have completed the enrollment. Be sure to include your Pharmacy Name, Vaccinations Integration, State, and Contact Information in the email body.
Note: OK-OSIIS will only onboard pharmacy locations that are registered for the COVID Pandemic Provider Enrollment.
Step 2: Document Immunizations
- Please continue to document immunizations in your registry portal and the Vaccinations tool while Outcomes works on your pharmacy’s integration.
Step 3: Final Integration Steps
Outcomes will send you an email confirmation when your pharmacy location has been approved to go live to report to OK-OSIIS with your Vaccinations tool. Once your pharmacy is in production, you will no longer need to report vaccines on your state registry website manually.
