How to enroll your pharmacy with the Philadelphia (PHI-PhilaVax) Registry

Step 1: Registration

    Once your registration is complete, please send an email to iisforms@prescribewellness.com with the completed Clinic Enrollment Form and User Confidentiality Form attached and the following subject line: [Pharmacy Name] | PhilVax | Clinic Enrollment Form Completed.

    Step 2: Document Immunizations

      • Please continue to document immunizations in your registry portal and the Vaccinations tool while PrescribeWellness works on your pharmacy’s integration.
      • If you need training on the PrescribeWellness Vaccinations tool, please visit PrescribeWellness Success Center and register for a live workshop.

      Step 3: Final Integration Steps

      Once the state registry approves your location to move forward into Production, the PrescribeWellness team will send you a confirmation email with your go-live date. From then on, your pharmacy can start reporting to PhilaVax with your Vaccinations tool.
