Step 1: Enrollment

• Please fill out and submit the below form to enroll:
  • USIIS Provider Facility Enrollment Agreement
• Once complete, please send a confirmation email to iis@prescribewellness.com. Be sure to include your pharmacy name, state, and contact information.

Confidentiality Agreements

Please review and sign the below confidentiality agreement. Each user needing access to the system must complete, sign, and submit an agreement:

1. USIIS Confidentiality and Security Policy
2. USIIS User Confidentiality and Security Agreement

Step 2: Documentation in the Vaccinations Tool and Testing

• Please continue to report immunizations to UT-USIIS as well as within your Vaccinations tool while PrescribeWellness works on your pharmacy’s User Confidentiality and Security Agreement
• If you need training on the PrescribeWellness Vaccinations Tool, please visit PrescribeWellness Success Center and register for a live workshop.

Step 3: Final Steps

Utah Status Check

• The Utah Registry will provide PrescribeWellness with testing credentials
• Once credentials are received, your pharmacy will go into Active testing
• UT-USIIS will review the most recent vaccines and check to see that all required information has been submitted properly
• After approval, UT-USIIS will confirm that your pharmacy is ready for Production.
