      How to enroll your pharmacy with the West Virginia (WVSIIS) Registry

      Step 1: Registration

      Please complete and submit the below application:

      1. WVVIIS Web Application
        1. Complete the New User Enrollment Form and submit directly to WVSIIS
        2. Please also return a copy to iisforms@prescribewellness.com with the following subject line: [Pharmacy Name] / Vaccination Tool / WV – Attached Copy of New User Agreement

      Step 2: Document Immunizations

        • Please continue to document immunizations in your registry portal and the Vaccinations tool while Outcomes works on your pharmacy’s integration.
          • You can also find pre-recorded webinars HERE.

        Step 3: Final Integration Steps

        1. Once Outcomes receives a copy of your New User Agreement, we will begin working with the registry on your interface connection.
        2. Outcomes will send an email confirmation when your pharmacy location has been approved to go live to report with WV-SIIS. Once approved, your pharmacy will be able to report solely through the Outcomes Vaccinations tool.
