How to enroll your pharmacy with the West Virginia (WVSIIS) Registry
Step 1: Registration
Please complete and submit the below application:
- WVVIIS Web Application
- Complete the New User Enrollment Form and submit directly to WVSIIS
- Please also return a copy to iisforms@prescribewellness.com with the following subject line: [Pharmacy Name] / Vaccination Tool / WV – Attached Copy of New User Agreement
Step 2: Document Immunizations
- Please continue to document immunizations in your registry portal and the Vaccinations tool while PrescribeWellness works on your pharmacy’s integration.
- If you need training on the PrescribeWellness Vaccinations tool, please visit PrescribeWellness Success Center and register for a live workshop.

Step 3: Final Integration Steps
- Once PrescribeWellness receives a copy of your New User Agreement, we will begin working with the registry on your interface connection.
- PrescribeWellness will send an email confirmation when your pharmacy location has been approved to go live to report with WV-SIIS. Once approved, your pharmacy will be able to report solely through the PrescribeWellness Vaccinations tool.
