      How to enroll your pharmacy with the Wyoming (WY-WyIR) Registry

      Step 1: Registration

      Please complete the below enrollment forms:

      1. WyIR – Onboarding Discovery Form
      2. WyIR – Onboarding EMR Demonstration Form
      3. WyIR – Click HERE to request new user access

      Once complete

      • Please email or fax copies of your completed forms to: iisforms@prescribewellness.com / 888-790-0767
      • PrescribeWellness will then reach out to WyIR with your pharmacy’s enrollment forms to initiate integration

        Step 2: Document Immunizations

          • Please continue to document immunizations to WY-WyIR and the Vaccinations tool while PrescribeWellness works on your pharmacy’s integration.
          • If you need training on the PrescribeWellness Vaccinations tool, please visit PrescribeWellness Success Center and register for a live workshop.

          Step 3: Final Integration Steps

          PrescribeWellness will notify your pharmacy with a confirmation email once your pharmacy has been approved for production and is live to report to WY-WyIR with the Vaccinations tool.
